Hotel Oakland Village Partner, Family Bridges, Inc. awarded new contract for a pilot program with Alameda Alliance for Health to be a ‘Community Based-Care Management Entity!’

We are excited to announce that Family Bridges, Inc., our Village partner, has signed a contract for a new pilot program with Alameda Alliance for Health to be a ‘Community Based-Care Management Entity (CB-CME).” This pilot program will be on-site at Hotel Oakland Village and offered to all Village residents! Community Based Care Management Entities will be the frontline providers of services at Health Homes. This CB-CME contract will allow Family Bridges to serve Alameda County’s most frail Medi-Cal seniors and persons with disabilities, who are highest utilizers of ER and acute care, including those with behavioral health issues, and homelessness. The primary focus of Family Bridge’s service as a Community Based-Care Management Entity will be enhanced case management.

This is a pilot program to get ready for the Health Home Program, due to roll out in 2019 in Alameda County. Family Bridges is one of five sites chosen as a Community Based-Care Management Entity (CB-CME).
